Cardiometabolic, Respiratory
& Chronic Health Conditions
At True Potential, we support people living with cardiometabolic and chronic health conditions through structured, individualised exercise programs. Our approach focuses on improving cardiovascular fitness, metabolic health, physical activity levels, preserving muscle mass, and building confidence in movement to support long-term outcomes.
Exercise is prescribed and progressed based on your medical history, risk factors, injury history, and current capacity. Programs are designed to be safe, adaptable, and sustainable—supporting lasting behaviour change rather than short-term fixes.

Over half of Australians have at least one chronic health condition, highlighting the importance of proactive exercise and lifestyle management. Strength training plays a key role in supporting cardiometabolic health by improving insulin sensitivity, energy regulation, and muscle metabolism, helping to manage blood glucose and reduce disease risk.

Less than 30% of Australians meet physical activity guidelines, which recommend 150–300 minutes of aerobic activity per week alongside at least two strength-training sessions. To meaningfully improve health and manage chronic conditions, we aim to progress beyond these minimum levels gradually and safely, working at your pace to reduce injury risk, avoid overwhelm, and fit exercise into your lifestyle. Understanding your baseline allows us to personalise progression and build resilience through structured, progressive exercise.
